Important Announcement

Elder Creek families,

This is Principal Brandon Frink with a short announcement. I hope everyone has a restful and fun break. With that said, I do have an important message to relay to you. As many of you have heard, all students must complete the form reporting their vaccination or exemption status by 11/30/21. Thanksgiving break is a great time to schedule your student’s vaccination, meet with a physician if filing an exemption and get your paperwork completed. SCUSD is also posting school site and community vaccination clinic opportunities on the website.

Students Aged 11 or Younger:

  • SCUSD does not currently require this age group to be vaccinated. All you need to do by Tuesday, November 30th is:

  1. Submit the student form at scusd.edu/reportvaccine, selecting the “Not Yet Required (age 11 or under)” option.

  2. Register your student (if not yet fully vaccinated) for COVID-19 testing at scusd.edu/covid-19-testing. NOTE: You only need to register and consent for COVID-19 testing one time a year. If you have already registered your student there are no further steps required.

Students Aged 12 or Older

  • SCUSD requires vaccination. Some exemptions may apply. All you need to do by Tuesday, November 30th is:

  1. Complete the student form at scusd.edu/reportvaccine and submit your student’s vaccination record or valid exemption.

  2. Register your student (if not yet fully vaccinated) for COVID-19 testing at scusd.edu/covid-19-testing. NOTE: You only need to register and consent for COVID-19 testing one time a year. If you have already registered your student there are no further steps required.

For more information about our district’s vaccination efforts, please visit scusd.edu/vaccinations.

In case you missed it, SCUSD had a Town Hall webinar to answer questions about our vaccination requirements and explain where to find all the forms we’ll need them to complete. You can watch the webinar in its entirety here.

Thank you and have a restful Thanksgiving break. We are grateful to each and every one of our students, staff and families. We look forward to seeing your student when school resumes on Monday, November 29th.
